A new Ealing pilot project supporting vulnerable older residents has reduced the risk of hospitalisation by 13% in just six weeks.
The initiative brings together Ealing Council, Ealing Primary Care, North West London Integrated Care Board and West London NHS Trust to provide joined-up support for older people living with frailty.
Using the North West London Frailty Risk Radar, healthcare teams identify patients most at risk of deterioration or hospital admission using primary care, emergency and hospital data.
The patients are then reviewed by a multi-disciplinary team including GPs, geriatricians, community nurses, pharmacists and mental health specialists.
The pilot began in February 2026 and has reviewed around 30 cases so far.
Dr Anna Down, Ealing Partnership GP lead for Older People services, said the team carries out “joined-up” assessments covering health, mobility, medications and home situations to reduce the likelihood of patients’ conditions worsening.
Dr Nevil Cheesman, Old Age Consultant Psychiatrist at West London NHS Trust, said the early results showed “the power of bringing professionals together around the needs of one person”.
